Medicare Benefits Counseling & Assistance	State Health Insurance Program (SHIP) Assistance with Medicare/Medicaid and supplemental insurance. We help with finding the best Part D (prescription) plan to meet your needs. Senior Medicare Patrol (SMP) This is a project that recruits volunteers to learn about the Medicare system and teaches beneficiaries how to read their insurance statements. Indigent Burial	A completed application must be received at the Commissioners' Office prior to burial or cremation and must meet indigent criteria. The deceased must be a resident of Flathead County. Please contact the Commissioners' Office (phone: 758-5503) or contact the funeral home for an indigent burial application. When submitting an application you must also submit the statement cost from the funeral home. We do not pay for funeral services, graveside services, memorial services, embalming, transportation of remains, caskets, urns, etc. There is a flat fee paid for either cremation or burial. Ombudsman Program	Advocacy for all residents of long-term care facilities (nursing homes and assisted living facilities), assisting them in resolving problems or complaints. Marriage License	All persons applying for a Montana marriage license must be in a position to provide proof of age - such as a valid driver's license or birth certificate - and must be single persons. This office does not require copies of dissolution papers, however, since the sworn application regarding your status as a single person is sufficient. Tobacco Use Prevention	As a contractor of the Montana Tobacco Use Prevention Program, Flathead City-County Health Department’s approach toward tobacco use prevention includes the following: Prevent tobacco use among youth Eliminate disparities related to tobacco use among populations Eliminate exposure to secondhand smoke Encourage people to quit Services Schedule a presentation about secondhand smoke exposure, tobacco, or e-cigarettes. Braveheart Chaplain Ministry	Besides responding to traumatic situations and providing emotional and logistical support to citizens in need, a chaplain to emergency services personnel also provides support to first responders in their daily duties. CARES Reporting Form	Cardiac Arrest Registry to Enhance Survival (CARES) was developed to help communities determine standard outcome measures for out-of-hospital cardiac arrest (OHCA), allowing for local quality improvement efforts and benchmarking capability to improve care and increase survival. The program seeks to save more lives from OHCA; strengthen collaboration between 911 centers, first responders, emergency medical services (EMS) agencies, and hospitals; provide a simple, confidential process for assessing patient outcomes in compliance with HIPAA; offer technical assistance to help community leaders identify and prioritize opportunities to improve EMS performance, and generate annual national and statewide reports for benchmarking capability. Communicable Disease	Communicable diseases are transmitted from one person to another through a variety of ways including direct contact, contact with a bodily fluid or blood, or indirectly by a vector (i.e. tick or mosquito).
