"Participant" as used in this document shall refer to the individual taking part in the associated activity. As the Participant or as the legal parent/guardian of the Participant(s) listed below I hereby agree to abide by all rules and regulations of the sports programs administered by Flathead County Parks and Recreation.

I understand participation in this activity requires the assumption of inherent risks of injury associated with the activity. Participation in the activity may subject Participant to minor injury or serious neck and spinal injuries, complete or partial paralysis, brain damage, other serious injuries to bones, joints, ligaments, and tendons, serious injury to other areas of the body, general health and well-being or death.

I acknowledge engaging in this activity may require a degree of skill and knowledge and acknowledge responsibilities of the Participant. I acknowledge the staff of Flathead County Parks and Recreation has been available to fully explain to me and I fully understand the nature and physical demands of this activity and the inherent risks, hazards, and dangers associated with this activity.

I certify Participant is fully capable of engaging in this activity and suffers from no mental or physical disabilities which prevent Participant from safely participating in this activity.

I acknowledge Flathead County Parks and Recreation makes no warranty as to the design, manufacture, maintenance, condition or fitness of any facilities or equipment Participant will be using.

I agree to accept complete responsibility for myself, including all minor children in my care, custody, and control, in matters of any property damage, physical injury, death or loss which may result from any activity associated with games, practices or travel to or from the activity from any dangers or risks identified herein and those inherent risks and dangers not specifically identified, and as a result of my negligence in participating in this activity.

I agree to release, indemnify, defend and hold harmless Flathead County Parks and Recreation Department and any of its sponsoring bodies, agents or any group, individual or agency associated with the sponsorship of this activity from all liabilities, losses, costs, damages, claims or causes of action of any kind or nature whatsoever, and expenses, including attorney's fees, arising or claimed to have arisen out of personal injuries or death, or property damage or loss, sustained by Participant as a result of any cause whatsoever, including but not limited to Participant's own conduct.
