What is included in the WISEWOMAN program?
The WISEWOMAN program consists of three (3) free heart disease screenings conducted with a Logan Health healthcare provider. The screenings will include:
- Measuring your blood pressure, cholesterol, blood sugar, height, weight, and waist.
- Talking about any heart disease risks you have, your usual eating habits, how active you are, and if you smoke.
- Talking about medicines you take to lower blood pressure, cholesterol, or blood sugar.
- Talking about any issues you might have with housing, transportation, or access to food.
- Learning about how to lower your heart disease risk.
- Referring you, if needed, to lifestyle management programs or services or help manage heart disease risk or your social needs.
What clinics are participating in WISEWOMAN?
The WISEWOMAN program is currently available in 3 clinics:
- Logan Health Bigfork: 8299 Montana Highway 35
- Logan Health Lakeside: (Located in the Lakeside Town Center) 306 Stoner Loop
- Logan Health Columbia Falls: 1850 9th St W, Columbia Falls, MT 59912
*Please note, you do not already have to be a Logan Health patient to enroll in WISEWOMAN; however, once you are in the program you will have to complete your appointments at one of the participating clinics.
If you are interested in enrolling in the WISEWOMAN program, you will first need to fill out our secure online Informed Consent Form. Once we receive a completed form, a WISEWOMAN staff member will reach out to you via phone call to complete the next steps of your enrollment. Our staff will also help to schedule your first WISEWOMAN appointment for you.
Yes! Patients who meet the eligibility requirements can be enrolled in both programs at the same time. Please note that you may have to fill out two separate enrollments.
The appointment will be very similar to a regular doctor's visit where your blood pressure will be taken, and staff will ask you various questions about your health.
Healthy behavior programs are different lifestyle sessions or classes designed to improve health and well-being. These are guided by coaches in your community who are trained in each program. There are different types of healthy behavior programs in Montana. These can include the National Diabetes Prevention Program, the Montana Tobacco Quitline, and Walk With Ease.
Do I Qualify?
Eligibility for women is based on age, income, and insurance.
Age: 35-64
Insurance: No insurance, insurance with a deductible of $250 or more, or coverage only through IHS/Tribal Health
Income: At or below 250% of the Federal Poverty Level (see income guideline chart below).
